Behavioural, Emotional or Mental Health Pathway
Aims and objectives of service
This pilot service is also known as Children and Young People with Behavioural, Emotional or Mental Health Needs and BEMH Nottingham. It aims to support children and young people with behavioural emotional, or mental health needs, and their parents/carers.
This service provides early and ongoing care planning and support for children and young people with behavioural, emotional, or mental health wellbeing needs. It includes inter-agency packages of care to support young people and their families and carers as they reach adulthood. It co-ordinates involvement between different agencies and promotes partnership working with parents/carers and the children and young people.
The aim is for intervention and prevention services to be in place at the earliest opportunity, ensuring children, young people and their families receive the most appropriate support to meet their needs.
The service will accept online or phone referrals to Health and Care Point, (integrated with current Child and Adolescent Mental Health Services Single Point of Access) from GPs, community professionals and specialist services where there is evidence of behavioural, emotional, or mental health needs.
The service is available to all children, young people (0-24) and their families/carers who are registered with a NHS Nottingham City GP.
