Care Homes Nursing Team
Aims and objectives of service
The aim of this service is to provide a dedicated nursing service to patients registered with a Nottingham City GP who live in a residential care home.
This service supports those living in residential care homes who have complex health problems and aims to maintain and maximise their independence by sourcing and delivering a range of health and social care services.
Support and advice is available via the telephone and through visits.
Patients who are seen by the service include: short-term acute patients; those with long term conditions; patients who develop Grade Three or Four pressure ulcers; patient who receive a continuing care package and those affected by seasonal flu.
The service has close working relationships with a range of services, clinicians and health professionals including: the Community Nursing and Rehabilitation Service; Specialist Nurse Practitioners in care homes; the end of life team, rehabilitation and occupational therapy services in care homes, Nottingham City Council and GPs.
Days/hours of operation
The service is available between 8.30am and 5pm Monday to Friday. Nursing staff are also available during these hours at weekends.

The service is available to adults registered with a GP practice in Nottingham City, living in a residential care home and with an identified nursing need.
