Continence Prescription Service

Aims and objectives of service

This pilot service, currently extended to 2016, aims to provide a timely, efficient, and tailored prescribing service for patients who use continence products. Through a centralised prescribing model across the city and county Clinical Commissioning Groups (CCGs) it aims to create a good experience for patients and GPs by ensuring appropriate use of continence products can also prevent avoidable hospital admissions.

Service description

Specialist continence nurse prescribers, with clinical expertise, ensure that appropriate continence appliances are prescribed for patients according to their individual clinical needs.

Prescriptions will be sent to either the pharmacy, distribution appliance centre or address of the patients choice.

The specialist nurses regularly review and monitor all patients referred to the service .

The service also offers advice and support to patients, carers and healthcare professionals with regard to the appliances used and best management of their continence. A specialist catheter outreach nurse can provide support to community nursing staff with complex catheterisations to help avoid hospital admission. Support is also provided to patients using intermittent self catheterisation and / or urinary sheaths.
