Community Evening and Night Nursing Service
Aims and objectives of service
This service aims to support people who are housebound and have immediate or long term health needs during evenings and nights that cannot wait until normal working hours.
Support provided under the Community Evening and Night Nursing Service is targeted at helping people with specific needs to maintain and maximise their independence. In particular the core service will assist with prevention of falls; continence issues; diabetic monitoring; end of life/palliative care; administration of medications; nutrition and skin and wound care.
The service is provided to people who are housebound and in response to clinical needs. A housebound citizen is a person who, as a result of physical or psychological disability is unable to leave their home via regular or adapted transport to attend established health care providers within their community.
Days/hours of operation
The service operates every night with evening core hours being 4.30pm to midnight and night core hours being 11pm to 9am.
Referrals are accepted via telephone or fax.
The Community Evening and Night Service is available to those who are registered within NHS Nottingham City Clinical Commissioning Group and patients who are registered with a GP in the South of Nottinghamshire, Nottingham North & East, Nottingham West and Rushcliffe CCGs.
