Continuing Care & Funding Nursing Care

Aims and objectives of service

The objectives of this service are to assess patients who have been identified as likely to be eligible for continuing healthcare, or who are placed in a nursing home and are eligible for receipt of funded nursing care. It reviews patients who receive fully funded continuing healthcare, including fast track funding, joint funded care packages, or funded nursing care and case managed patients in receipt of NHS funded care.

Service description

The continuing care assessment and review process sits over both primary and secondary care services as well as social care. Key stakeholders are acute trusts, Mental Health and Learning Disability Trusts, providers of care - such as care homes and agencies - local authorities, GPs and community nursing teams.

Children under the age of 17 are excluded from this service.

Population covered

Registered population of NHS Nottingham City Clinical Commissioning Group (i.e. registered with a City GP regardless of home address).
