Adult Community based Speech & Language Therapy
Aims and objectives of service
This service provides assessment, rehabilitation and therapies for patients aged 18 and over who have communication and swallowing disorders. It provides self-care support and advice to relatives, together with an integrated, holistic view of the patients care, the service will identify and co-ordinate appropriate specialist help when required.
The service is available to patients with ongoing communication and swallowing disorders as a result of a range of conditions including stroke, head injury and degenerative conditions such as dementia, Parkinson’s Disease and Multiple Sclerosis. It also work with patients on the end of life care pathway and residents in care homes and provides training for care home staff.
Patients can be seen in clinics, health centres, community settings or their homes - wherever is most appropriate. The service provides specialist help on a one to one or group setting, integrating treatment with the patient’s existing care.
The service also provides Professional Speech Language Therapy (SLT) support, training and advice to health and social care professionals including Assistant Practitioners and Multi-disciplinary Care Teams; agencies, carers and self-help groups to enhance patient care.
The SLT team works with and alongside the Community Nursing and Rehabilitation Teams, Adult Specialist Services, Community Nutrition and Dietetics, GPs and practice staff, local authority social services, voluntary groups, residential and nursing homes and other service providers.
Days/hours of operation
The service is available Monday to Friday between 8.30am and 5pm.
Referrals are received by Nottingham City Health Care Point from: GPs; community rehabilitation, nursing and specialist services staff; hospital discharge units; care homes; in-patient speech and language therapists; secondary care and self-referral.
Patients registered with a GP within Nottingham City.
