Urgent Care Interim Homecare Service (short term)
Aims and objectives of service
This service provides interim homecare support to people discharged by the Crisis Response Service and identified as requiring ongoing reablement support in the community until the appropriate care package is in place.
The service provides short term (up to eight weeks) reablement support to Nottingham City residents in their own home following discharge from the Crisis Response Service. The team assesses service users on an ongoing basis to ensure that the care package offered is flexible to their individual needs. The patient's care plan or achievement of goals document is updated on a daily basis. The service offers a planned, structured and responsive service, which offers both daytime and out of hours provision including short sitting services and overnight stays.
The service accepts referrals from the Nottingham City Crisis Response team.
The service is available to all informal carers of any age registered with a NHS Nottingham City Clinical Commissioning Group GP.
