Nottingham CityCare Partnership is delighted to announce its success in securing the tender for a new county-wide Care Navigation service; a key milestone in the delivery of proactive, community-based care for residents across Nottingham and Nottinghamshire. 

Following a successful tender, CityCare will expand its Care Navigation model from December 2025 to cover the entire Integrated Care System (ICS) footprint, building on over a decade of experience delivering high-quality navigation services in the city. 

The new service is designed to support the ICS Frailty Programme and is fully aligned with the Integrated Care Strategy and NHS Joint Forward Plan. It will focus on identifying and supporting people living with moderate to severe frailty (score of 6+), as well as those with long-term conditions such as cardiovascular disease, COPD, coronary heart disease, and diabetes. 

Chief Executive Nic Atkinson said: 

“This is a significant step forward in our mission to deliver compassionate, proactive care that meets the needs of our ageing population. Our Care Navigators will work closely with GPs, Integrated Neighbourhood Teams, acute and community clinical staff, social care and voluntary sector partners to ensure people receive the right support at the right time.” 

The service will use clinical frailty scoring and condition-based criteria to identify vulnerable patients and connect them with personalised support. Care Navigators will be embedded within local teams and will play a vital role in reducing hospital admissions, improving patient outcomes, and enhancing quality of life.

The new model marks the beginning of a broader transformation in how care is delivered in the community, with the Care Navigation model expected to evolve to support additional pathways in the future.
