We support adults who have osteoporosis, weak bones, or who may be at risk of breaking a bone (fracture).

You do not need to have already had a fracture to be referred to us. We help people before and after a fracture, to reduce the risk of future breaks and support long‑term bone health.

Our aim is to help you:

  • Keep your bones as strong as possible
  • Reduce your risk of fractures
  • Manage osteoporosis confidently at home

What Is a Fragility Fracture?

A fragility fracture is a broken bone that happens after a minor fall or simple injury, such as:

  • Falling from standing height
  • Slipping or tripping
  • A small knock that wouldn't normally break a healthy bone

These fractures can happen because bones are weaker, often due to osteoporosis.Common areas include the hip, spine (back), wrist, and upper arm.

You will be supported by a small, friendly specialist team, including:

  • Specialist nurses
  • Bone health treatment nurses
  • Administrative staff

How We Work

We follow national and local guidance and use the best available evidence to make sure you receive safe, effective care. We work closely with your GP, hospital teams, and local services.


What We Can Help With

We offer:

  • You may have been prescribed oral medication for osteoporosis by your GP, such as alendronic acid, risedronate, or ibandronate. Your doctor may refer you to our service for ongoing management and monitoring while you are taking this medication. This monitoring may include blood tests and scans
  • Bone health assessments, including checking your fracture risk
  • Referrals for bone scans (DXA) if needed
  • Medicines for bone health, including monitoring and follow‑up
  • Home visits for some osteoporosis treatments given by injection or infusion
  • Advice and support for living well with osteoporosis
  • Personalised care plans based on your individual risks and needs

We aim to keep your care close to home wherever possible and reduce unnecessary hospital visits.


Who Can Be Referred

You may be referred to us if you:

  • Are aged 50 or over and have osteoporosis or osteopenia
  • Have had a fragility fracture
  • Have been identified as at medium or high risk of fracture
  • Are registered with a Nottingham City GP practice

People under 50 usually need to be referred to hospital specialists instead.

Referrals can be made by:

  • Your GP
  • Hospital staff
  • Other healthcare professionals involved in your care

Bone Health Specialist Service 

The Bone Health Specialist Service are a group of highly skilled and experienced nurses in the field of osteoperosis and bone health working for Nottingham CityCare Partnership.

The team see patients in their own homes or care homes. They provide Nurse Led assessment and intervention to support patients and GPs with the management of Bone Health.

This leaflet aims to help you understand more about the Bone Health Specialist Service and enables you to make informed decisions about your care and treatment.

About your care and treatment

Denosumab is a monoclonal antibody that binds to the RANK ligand protein produced by osteoblasts that then normally activates and develops osteoclasts. It inhibits this action and as such decreases bone resorption.

How will a homecare service benefit me?

At regular intervals, a specialist nurse will deliver your medicine to you at home (or another place that is convenient for you) and administer it. This will save you time and inconvenience by avoiding the need to visit hospital to receive your treatment.

It is important that you engage with regular appointments with the nurse if required whilst you are receiving home treatment to ensure that your health is monitored effectively and the appropriate medication can be prescribed. Not attending your appointments could lead to your treatment at home being interrupted.

What are my responsibilities with homecare?

If you are using the specialist service we ask that you:

• Are able to engage with regular appointments

• Provide a home or other alternative address where the medicine can be administered safely

• Provide a contact number for messages

• Keep us informed if your contact details change

• Answer any phone call from the specialist nurses

• Be present to receive the treatment on the scheduled day and time

What if I have a problem?

The service has a direct telephone number to call should you have any queries. This line is answered Monday to Friday excluding weekends and bank holidays. Should the nurses be out of the office, please leave a message which will be answered within 48 hours.

Confidentiality

The Bone Health Specialist Nurses are bound by confidentiality rules and must comply with the Data Protection Act 1998. They will not give information about you to anyone else or discuss your treatment with anyone except the hospital staff and your GP who provides care for you

What is Zoledronate?

Zoledronate is one of the bisphosphonate drug treatments which are used to reduce the risk of broken bones in people with osteoporosis. It is given as an intravenous infusion (drip) approximately every 18 months.The drug is given slowly, typically over 15 minutes via a needle inserted into a vein. If Zoledronate is prescribed and you have recently broken your hip, it is recommended that Zoledronate is given two or more weeks after your hip repair surgery.

What is ‘Zoledronate infusion at home’?

Not only will the medicines be delivered to your home; a nurse from the specialist service will visit and administer the drug for you at home, in the same way as you would normally receive treatment in hospital.The nurse will carry out some observations before and after drug administration to ensure that you have no problems during or following the drug infusion. This service is offered to you free of charge.

What are my responsibilities with homecare?
