We are a non clinical team who receive inbound calls from patients, patients’ families and health care professionals who are under the care of the community nurse, community matron and the evening & nights' service.
We provide the first level of triage to these callers using using questionnaires embedded within our clinical software, SystmOne. We book visits and signpost callers as appropriate, escalating urgent cases onto a clinical responder or the emergency services as appropriate. We book visits, deal with queries and signpost to appropriate services and carry out admin for the Evening and Night Service.
Patient are given a booklet detailing the process of contacting the hub and the circumstances for doing so upon their first visit from a member of the Neighbourhood Team.
