Our specialist nursing team provides complex healthcare and advice to people who are rough sleeping or vulnerably housed. We deliver care through drop-in clinics, temporary accommodation and clinical street outreach. We also provide ongoing support following complex hospital discharges and work with hospital teams through in-reach services.
Operating Hours
The service operates Monday to Friday, from 08:00 to 18:00. We also provide weekly early-morning clinical street outreach outside these hours.
Our Team
The team comprises of:
- One Clinical Nurse Specialist and SMD Clinical Lead
- Two SMD Nurse Specialists
- One SMD Specialist Assistant Practitioner
- One SMD Specialist Care Coordinator
Support We Provide
Our main areas of support include:
- Person-centred nursing triage
- Enhanced clinical assessment
- Assessment and treatment of minor illnesses and injuries
- Dressings and complex wound care
- Blood tests
- Removal of cannulas, clips and sutures
- Pregnancy testing
- Support with complex hospital discharge and follow-up care
Access to Ongoing Care
At the first consultation, we check that each patient is registered with a local GP practice and community pharmacy so they can access appropriate treatment.
Partnership Working
We work closely with GP practices to maintain continuity of care and escalate complex case management when needed. We also support patients to access mainstream healthcare services.
