We will support you if you have a long-term condition or are recovering from an episode of ill health where you are unable to access services outside of your home. Our service is made up of Assistant Practitioners, Registered General Nurses, Senior Nurses, District Nurses and Community Matrons.
We will help you with the following:
- Diabetes care
- Wound management
- End of life care
- Continence assessments
- Medicines management
- Case management for long term conditions
We provide nursing for housebound patients that have an acute, short-term nursing need, for example after an operation. We support by providing advice about nursing needs to patients, carers and home care providers.
A community matron oversees this process and will identify where additional support may be needed for long-term conditions and to avoid hospital admissions, or support with end of life care. This includes medication management and prescribing which will be done in partnership with your GP.
We carry out a full assessment at the first meeting in order to identify your needs and with your consent will refer onto further appropriate services.

Patients who are already known to the service can contact the Nursing Triage Hub on 0115 8838151. This number is available 24 hours a day, 7 days a week.
